Taxpayer Instructions
In order to make sure that we can prepare your return for you, please read the taxpayer instructions below.
There are two forms that MUST be filled out before we can prepare your taxes for you. These forms ask for information about you and everyone else on your tax return. They also ask about all of your sources of income and any expenses that you had that will help us in preparing an accurate return for you.
If you have any deductions related to the new tax laws, such as tip, overtime, & self employment income, or you are claiming an interest deduction for a new vehicle purchased, you must fill out the worksheets listed below. We will not be able to prepare your return if you do not have the appropriate worksheet(s).
Fill out the forms online then print them out and bring them with you to your tax appointment.
If you are unable to print, the forms will be available at the front desk of the site where your appointment is scheduled, during their regular hours. They will not have the worksheets. You will need to visit the tax site, when we are there, to pick up worksheets from us.
Required Forms
Taxpayer Instructions – Read these instructions to make sure that you have all of the information we need in order to prepare your tax return. Also, check that we are able to prepare your return for you. There are some returns that we cannot prepare
Federal Intake Booklet – This form is 10 pages long. Please fill out the entire form, answering all the questions (except the areas in gray that are labeled for a “Certified Volunteer Preparer”). Fill out the form online, then print it out and bring it with you to your appointment.
Wisconsin Information Sheet – This form is two pages long. Fill out the form online, then print it out and bring it with you to your appointment.
